ABSTRACT

This study aimed to identify and determine the level of factors associated with the health-related quality of life (HRQoL) of oral health professionals working in Primary Health Care (PHC) in Brazil. A cross-sectional survey was carried out among 96 dentists and 65 dental assistants working in the Family Health Strategy (FHS) in six municipalities in the metropolitan area of Salvador (BA), Brazil. The HRQoL was evaluated using the 36-item Short Form Health Survey Questionnaire (SF-36). Higher mean scores in the SF-36 physical component summary were associated with younger age, post-graduate education in public health, and frequent leisure time. The mental component summary was associated with frequent leisure time, > 40 weekly working hours, suitable dental offices, satisfaction with colleagues, and satisfaction with working in PHC. The HRQoL level of oral health professionals working in primary health care in PHC in Brazil was comparable to those found in the available literature and associated with several modifiable factors. Incidentally, a literature search found few studies using the SF-36 to assess the HRQoL of oral health workers, and those found were produced outside the main scientific centers.


O objetivo deste estudo foi identificar e determinar o nível de fatores associados à qualidade de vida relacionada à saúde (QVRS) de profissionais de saúde bucal que atuam na Atenção Primária à Saúde (APS) no Brasil. Foi realizado um estudo de corte transversal com 96 cirurgiões-dentistas e 65 auxiliares em saúde bucal que atuam na Estratégia Saúde da Família (ESF) em seis municípios da região metropolitana de Salvador (BA), Brasil. A QVRS foi avaliada usando o 36-item Short Form Health Survey Questionnaire (SF-36). Escores médios mais altos no resumo do componente físico do SF-36 foram associadas a idade mais jovem, pós-graduação em saúde pública e tempo de lazer frequente. O resumo do componente mental foi associado a tempo de lazer frequente, > 40 horas semanais de trabalho, consultórios odontológicos adequados, satisfação com os colegas e satisfação em trabalhar na APS. O nível de QVRS dos profissionais de saúde bucal que atuam na APS no Brasil foi comparável aos encontrados na literatura disponível e associado a vários fatores modificáveis. Incidentalmente, uma pesquisa bibliográfica encontrou poucos estudos que utilizam o SF-36 para avaliar a QVRS de trabalhadores da saúde bucal; e os encontrados foram produzidos fora dos principais centros científicos.


El objetivo de este estudio fue identificar y determinar el nivel de los factores asociados a la calidad de vida relacionada con la salud (CVRS) de los profesionales de salud bucal que actúan en la Atención Primaria de Salud en Brasil. Se realizó un estudio transversal con 96 cirujano dentista y 65 auxiliares de salud bucal que actúan en la Estrategia Salud de la Familia en seis municipios de la región metropolitana de Salvador, Brasil. La CVRS se evaluó mediante el 36-item Short Form Health Survey Questionnaire (SF-36). Las puntuaciones medias más altas en el resumen del componente físico del SF-36 se asociaron con una edad más joven, títulos de posgrado en salud pública y tiempo libre frecuente. El resumen del componente mental se asoció con tiempo libre frecuente, > 40 horas de trabajo por semana, consultorios dentales adecuados, satisfacción con los colegas y satisfacción con el trabajo en la Atención Primaria de Salud. El nivel de CVRS de los profesionales de la salud bucal que actúan en la Atención Primaria de Salud en Brasil fue comparable a los encontrados en la literatura disponible y se asoció a varios factores modificables. Una búsqueda bibliográfica encontró pocos estudios que utilizan el SF-36 para evaluar la CVRS de los trabajadores de la salud bucal; y los encontrados estaban fuera de los principales centros científicos.

ABSTRACT

Aim: to analyze, between 2019-2021, the quantitative changes in dental emergencies (DE) in Brazil and its regions. Materials and Methods: Data collected in the Primary Care Health Information System (SISAB) from 2019 to 2021. The DE represented by dentoalveolar abscess (DAA) and toothache (TA). 2019 was the control (non-pandemic), and 2020-21 the exposure year. Durbin-Conover's Friedman and Post-Hoc tests used a significance level of 5%. The data's organization used the percentage difference to facilitate analysis. Results: For Brazil in 2021, the percentage difference with 2019 suggests that DAA (-2.16%, p=1.0) and TA (+14.94%, p=0.064) returned to values after fall of 2020. The South region, in 2020, had no decrease in DAA (-5.48%, p=0.436) and TA (+3.7%, p<0.001) in 2020, and an increase in both in 2021 (DAA: +26.86%, p<0.001; TA: +51.06%, p<0.001). Discussion: In 2021, in Brazil, limited elective access and resumption of DAA and increase in TA suggest worsening the oral health and quality of life. The DAA and TA results in the South region do not provide plausible evidence to understand the unchanged values in 2020 and the considerable increase in 2021. Conclusion: Regardless of the pandemic, elective access still struggles to offer universal acessing, equitable, and the need of investments are essentials to prevent public services from becoming just gateways for relieving pain and suffering.


Objetivo: analisar, entre 2019-2021, as alterações quantitativas nas urgências odontológicas (UO) no Brasil e suas regiões. Materiais e Métodos: Dados coletados no Sistema de Informação em Saúde da Atenção Básica (SISAB) no período de 2019 a 2021. A representação das UO foi pelo abscesso dento-alveolar (ADA) e dor de dente (DD). O ano de 2019 foi o ano de controle (não pandêmico) e 2020-21 os de exposição. Os testes Friedman e Post-Hoc de Durbin-Conover utilizaram nível de significância de 5%. Os dados foram organizados pela diferença percentual para facilitar a análise. Resultados: Para o Brasil em 2021, a diferença percentual com 2019 sugerem que o ADA (-2,16%, p=1,0) e a DD (+14,94%, p=0,064) retomaram os valores, após a queda de 2020. A região Sul, em 2020, não teve queda em ADA (-5,48%, p=0,436) e DD (+3,7%, p<0,001) em 2020, e aumento em ambos em 2021 (ADA: +26,86%, p<0,001; DD: +51,06%, p<0,001). Discussão: Em 2021, no Brasil, o limitado acesso eletivo e a retomada da ADA e aumento da DD sugerem piora na saúde bucal e na qualidade de vida. Os resultados de ADA e DD na região Sul não apresentam evidências plausíveis para compreender a inalteração de valores em 2020 e o considerável aumento em 2021. Conclusão: Independentemente a pandemia, o acesso eletivo ainda luta para ser universal, equânime e os investimentos precisam ser retomados para evitar que os serviços públicos se tornem em apenas portas de entrada de alívio dor e sofrimento.

Introducción: Cuba se ha caracterizado por enviar a diversos países delegaciones de salud, llamadas "misiones médicas". Sin embargo, el interés de los futuros profesionales de formar parte de esas delegaciones no se ha investigado. Objetivos: Caracterizar la disposición hacia la colaboración médica internacional e identificar los factores asociados en estudiantes cubanos de Estomatología. Métodos: Se realizó una investigación transversal, analítica de datos secundarios, en estudiantes de todos los años del curso académico 2018-2019, pertenecientes a ocho universidades. La variable dependiente fue el reporte de la disposición de ir a misiones médicas. Se calcularon las razones de prevalencia e intervalos de confianza al 95 por ciento. Resultados: Participaron 1174 estudiantes, de los cuales 830 (71 por ciento) estuvieron dispuestos a cumplir misiones médicas. Existió una mayor disposición hacia la colaboración médica conforme aumentaba la edad (RPa: 1,02; IC 95 por ciento: 1,00-1,04; p = 0,044); así como, en los estudiantes que reportaron presión familiar para estudiar la carrera (RPa: 1,17; IC 95 por ciento: 1,09-1,26; p < 0,001). Aquellos que reportaron buenas notas antes de ingresar a la universidad (RPa: 0,83; IC 95 por ciento: 0,77-0,91; p < 0,001), los de solvencia económica (RPa: 0,90; IC 95 por ciento: 0,90-0,98; p = 0,019) y los que estaban cursando el tercer y cuarto años académicos (RPa: 0,93; IC 95 por ciento: 0,88-0,97; p = 0,003) fueron los de menor disposición para colaborar; ajustado por el sexo y la universidad. Conclusión: Existe una elevada disposición hacia la colaboración médica internacional y está asociada a múltiples aspectos sociales y educativos(AU)


Introduction: Cuba has been characterized by sending health delegations to various countries, called "medical missions". However, the interest of future professionals in joining those delegations has not been investigated. Objectives: Characterize the disposition towards international medical collaboration and identify the associated factors in Cuban students of Stomatology. Methods: A cross-sectional research and secondary data analytics was carried out in students of all years of the academic year 2018-2019, belonging to eight universities. The dependent variable was the report of the willingness to go on medical missions. Prevalence ratios and 95 percent confidence intervals were calculated. Results: 1174 students participated, of which 830 (71 percent) were willing to fulfill medical missions. There was a greater willingness towards medical collaboration as age increased (RPa: 1.02; 95 percent CI 1.00-1.04; p = 0.044); as well as, in students who reported family pressure to study the career (RPa: 1.17; 95 percent CI 1.09-1.26; p < 0.001). Those who reported good grades before entering college (RPa: 0.83; 95 percent CI 0.77-0.91; p < 0.001), those of economic solvency (RPa: 0.90; 95 percent CI 0.90-0.98; p = 0.019) and those who were in the third and fourth academic years (RPa: 0.93; 95 percent CI 0.88-0.97; p = 0.003) were those with the least willingness to collaborate; adjusted by sex and college. Conclusion: There is a high disposition towards international medical collaboration and it is associated with multiple social and educational aspects(AU)

ABSTRACT

Abstract Objective: To assess the knowledge, dental anxiety, and expectations regarding dental services during the COVID-19 pandemic. Material and Methods: The respondents were Indonesian citizens above 18 years old. An online Google survey was administered using a structured questionnaire with a snowball sampling technique. Survey items comprised knowledge related to COVID-19, dental anxiety assessed using the modified DAS (Dental Anxiety Scale) and expectations regarding dental services using four dimensions of dental service quality. All questionnaires were tested for reliability and indicated acceptable and good agreement. The data were analyzed descriptively. Results: A total of 553 responses were analysed. Most respondents were female (72.9%), 76.7% knew of recommendations to postpone dentist visits and 86.8% knew methods of preventing COVID-19 transmission. More than 70% of respondents knew the precaution procedures in the dental office during COVID-19, and only 27.9% had moderate-severe anxiety. Most respondents' expectations regarding dental services during the pandemic era were related to the quality domain of reliability and responsiveness. Conclusion: Respondents knew about COVID-19 transmission and prevention, emergency conditions warranting a visit to the dentist and the procedures used at the dental office. Most respondents stated that they were not anxious about visiting a dentist during the pandemic. The respondents expect the dentist to provide sufficient information to improve oral health and treatment plan.

Aim The aim of this study was to evaluate the association between the sense of coherence (SOC) of parents/guardians and the use of dental services by children who are 12 years old. Methods This is a cross-sectional study conducted in the eastern zone of the city of Manaus, with 358 children enrolled in the 7th grade in the municipal public school system. Data collection was performed through self-administered questionnaires sent to parents/guardians, which contained questions about socioeconomic conditions, use of dental services by children and Antonovsky's SOC-13 scale. Data were examined by descriptive and bivariate analysis, using the Chi-square test, with a 5% significance level. Results Of the 358 children, 58.4% were female; in addition, 75.4% of the parents/guardians self-reported to be brown and 39.9% of them had a family income of ½ to 1 minimum wage. SOC was categorized as strong SOC and weak SOC by the median. The association between strong or weak SOC and the use of dental services in terms of frequency (p= 0.839) and reason for last visit (p= 0.384), was not significant. Conclusion It was concluded that SOC of parents/guardians and utilization of dental services by children were not associated.

ABSTRACT

ABSTRACT Objective: To analyze specialized dental care through access, demand and the work processes provided by the CEO's, using secondary data from the cycles of the PMAQ-CEO. Material and Methods: Cross-sectional study using public domain data. Variables of interest were selected from the external evaluation instrument data matrices and were grouped in categories. The categories were geographic distribution, structural, human resources, work processes and access and coordination. Results: The total of 932 CEO's were evaluated in 2014 and 1,042 in 2016, most of them type II, present in the Northeast region. In both cycles, the highest average of dentists worked in the endodontic specialty (2.4 professionals per CEO) and the lowest in oral medicine (0.8 professionals per CEO). Of the two cycles, 91.5% of the CEO's had a manager, 79.5% performed action planning activities and 74.5% realized internal self-assessment processes. There was an increase in the mean number of days to be seen at the CEO in all analyzed specialties (p<0.001) and 85.2% of the CEO's managers reported that there is a reference for oral cancer confirmed cases. Conclusion: Between the PMAQCEO cycles the number of CEO has increased, but there are still a big pent-up demand and the presence of care gaps. However, an expansion and improvement of the CEO's actions was evidenced, mainly related to the work processes.

ABSTRACT

O "Programa Nacional de Melhoria do Acesso e Qualidade da Atenção Básica ­ PMAQ-AB" foi lançado em 2011 pelo Ministério da Saúde, com objetivo de induzir a ampliação do acesso e a melhoria da qualidade da Atenção Primária em Saúde (APS) no Sistema Único de Saúde (SUS). O programa foi organizado em quatro fases que se complementam e conformam um ciclo contínuo: adesão e contratualização; desenvolvimento; avaliação externa e recontratualização. Com esse desenho, o PMAQ-AB buscou superar desafios para a qualificação da APS. O objetivo deste estudo foi descrever, analisar e compreender o desempenho das Equipes de Saúde Bucal (ESB) participantes do PMAQ-AB, por meio dos procedimentos odontológicos realizados, comparando os achados entre os ciclos e entre as regiões brasileiras. Utilizaram-se dados secundários do 2º e 3º ciclos do PMAQ-AB, ocorridos entre 2013 e 2019. Os dados foram obtidos na fase de avaliação externa, que envolveu entrevista com dentistas sobre o processo de trabalho das equipes. Este trabalho resultou em três artigos científicos. O primeiro e segundo artigo utilizaram dados do 3º Ciclo. No primeiro artigo realizou-se uma análise descritiva de 26 procedimentos odontológicos realizados na APS. Cada procedimento realizado atribuiu uma pontuação à ESB, sendo a pontuação final a soma do número de procedimentos realizados. Estas pontuações foram comparadas entre as regiões do país. Os resultados revelaram que as equipes realizaram, em média, 19,45 (±3,16) dos 26 procedimentos odontológicos analisados. As ESB do Sul, Sudeste e Nordeste realizaram um número maior de procedimentos, enquanto as equipes do Norte e Centro-Oeste realizaram, em média, menos procedimentos. No segundo artigo, foram avaliados 13 procedimentos (itens) relacionados a procedimentos odontológicos espontâneos, preventivos, cirúrgico, restauradores, protético e de câncer bucal. A Teoria da Resposta ao Item (TRI) foi utilizada para estimar as pontuações de desempenho das ESB, com base nos itens. A relação entre os escores de desempenho e as variáveis contextuais (Índice de Desenvolvimento Humano - IDH, e Índice de Gini) nas regiões brasileiras foi analisada por mapas temáticos e correlação de Pearson (p<0,05). Os procedimentos com maiores níveis de dificuldade e menos frequentemente realizados foram aqueles relacionados a próteses dentárias e monitoramento do câncer bucal. Os itens foram mais apropriados para discriminar ESB com baixo desempenho, e relativamente ineficazes para diferenciar aquelas com melhor desempenho. Os mapas temáticos mostraram uma relação direta com o IDH (p < 0,0001) e uma relação indireta com o índice de Gini (p= 0,0001). No terceiro artigo realizou-se um estudo comparativo do desempenho das equipes das ESB que participaram dos dois últimos ciclos de avaliação do PMAQ-AB, analisando 11 procedimentos odontológicos coincidentes nestes ciclos. A TRI foi utilizada para estimar os escores de desempenho das ESB em cada ciclo e esses escores foram comparados pelo teste de Wilcoxon (p ≤ 0,05), para o Brasil e regiões brasileiras. Os achados indicam que houve melhora nos escores de desempenho das ESB entre os dois ciclos no Brasil e nas regiões brasileiras. As regiões Nordeste, Centro-Oeste e Norte apresentaram as maiores evoluções. Porém, diferenças regionais ainda persistem, sendo necessário estruturar e melhorar as políticas públicas a fim de expandir e qualificar o processo de trabalho dos serviços de saúde bucal, promovendo uma melhor distribuição regional das ESB, a fim de reduzir as desigualdades.


The "National Program for Improvement of Primary Care Access and Quality - PMAQAB" was launched in 2011 by the Ministry of Health, aiming to induce the expansion of access and improvement of the quality of Primary Health Care (PHC) in the Unified Health System (SUS). The program was organized into four phases that complement each other and form a continuous cycle: adherence and contractualization; development; external evaluation and recontractualization. With this design, the PMAQ-AB sought to overcome challenges for the qualification of PHC. The objective of this study was to describe, analyze, and understand the performance of the Oral Health Teams (OHT) participating in PMAQ-AB, through dental procedures performed, comparing the findings between cycles and between Brazilian regions. Secondary data from the second and third cycles of PMAQ-AB, which occurred between 2013 and 2019, were used. The data were obtained in the external evaluation phase, which involved interviewing dentists about the work process of the teams. The first and second article used data from the third cycle. In the first article, a descriptive analysis of 26 dental procedures performed in the PHC was done. Each procedure performed attributed a score to the OHT, and the final score was the sum of the number of procedures performed. These scores were compared among the regions of the country. The results revealed that the OHTs performed, on average, 19.45 (±3.16) of the 26 dental procedures analyzed. The OHTs from the South, Southeast and Northeast performed a higher number of procedures, while the teams from the North and Midwest performed, on average, fewer procedures. In the second article, 13 procedures (items) related to spontaneous, preventive, surgical, restorative, prosthetic, and oral cancer dental procedures were evaluated. Item Response Theory (TRI) was used to estimate the OHT performance scores based on the items. The relationship between performance scores and contextual variables (Human Development Index - HDI, and Gini Index) in Brazilian regions was analyzed by thematic maps and Pearson correlation (p<0.05). The procedures with higher levels of difficulty and less frequently performed were those related to dental prostheses and oral cancer monitoring. The items were more appropriate to discriminate low performing OHT and relatively ineffective to differentiate those with better performance. The thematic maps showed a direct relationship with HDI (p < 0.0001) and an indirect relationship with Gini index (p= 0.0001). In the third article, a comparative study of the performance of the ESB teams that participated in the last two PMAQ-AB evaluation cycles was carried out, analyzing 11 dental procedures coincident in both cycles. IRT was used to estimate the performance scores of the OHT. After obtaining the performance scores of the second and third cycles, these scores were compared by the Wilcoxon signed-rank test (p ≤ 0.05) in Brazil and Brazilian regions. The findings indicate that there was improvement in the performance scores of the ESBs between the two cycles in Brazil and in the Brazilian regions. Northeast, Midwest, and North regions showing the greatest evolutions. However, regional differences still persist, and it is necessary to structure and improve public policies in order to expand and qualify the work process of oral health services, promoting a better regional distribution of the ESB in order to reduce inequalities.

ABSTRACT OBJECTIVE Measure the prevalence of use of dental services in the previous year and associated factors among 31-year-old adults from a birth cohort of 1982. METHODS This is a cross-sectional study that analyzed a birth cohort of 1982 from the city of Pelotas. In 1997, a systematic sample of 27% of the city's census sectors was defined and all households in these sectors were visited, where 1,076 15-year-old adolescents were interviewed. For the oral health studies, 900 of these individuals were randomly selected and followed up at 24 and 31 years of age. The study used data collected from 523 individuals in 2013 (at 31 years old). The outcome was visit to the dentist (use of dental services) in the previous year. Demographic factors (sex), socioeconomic factors (income, education), and oral health factors (reason and type of service, self-perception of oral health, dental pain and caries experience - DMFT) were used as independent variables. Prevalence ratios were estimated using Poisson regression. RESULTS The prevalence of use of dental services in the previous year was 55.3% (95%CI: 51.0-59.5%). In the adjusted analysis, the reason and type of service, self-perception of oral health, and DMFT were associated with the outcome. A stronger association was found with use of dental services in individuals who visited for prevention and used the private service, who were satisfied with their oral health, and who had more caries experiences. CONCLUSION 55.3% of the cohort sample used dental services in the previous year. Individuals who visited the dentist of private service for preventive reasons, who were very satisfied with their oral health, used these services in a higher proportion. In addition, a higher DMFT index also led to higher use of services.


RESUMO OBJETIVO Mensurar a prevalência de uso de serviços odontológicos no último ano e os fatores associados em adultos de 31 anos pertencentes à coorte de nascimentos de 1982. MÉTODOS Estudo transversal, aninhado na coorte de nascimentos de Pelotas de 1982. Em 1997, uma amostra sistemática de 27% dos setores censitários da cidade foi realizada e todos os domicílios desses setores foram visitados, onde 1.076 adolescentes de 15 anos foram entrevistados. Para os estudos de saúde bucal foram sorteados aleatoriamente 900 desses indivíduos, que foram acompanhados também aos 24 e 31 anos. O estudo utilizou dados coletados de 523 indivíduos em 2013 (aos 31 anos). O desfecho foi a ida ao dentista (uso do serviço) no último ano. Fatores demográficos, (sexo), socioeconômicos (renda, escolaridade) e de saúde bucal (motivo e tipo de serviço da consulta, autopercepção de saúde bucal, dor e experiência de cárie - CPOD) foram utilizados como variáveis independentes. As razões de prevalência foram estimadas usando a regressão de Poisson. RESULTADOS A prevalência de uso de serviços odontológicos no último ano foi de 55,3% (IC95%: 51,0 -59,5%). Na análise ajustada, o motivo e tipo de serviço da consulta, a autopercepção de saúde bucal e o CPOD foram associados ao desfecho. Foi encontrada maior associação com a utilização de serviços odontológicos em indivíduos que visitaram por prevenção e usaram o serviço privado, satisfeitos com a sua saúde bucal e que tinham maior experiência de cárie. CONCLUSÃO 55,3% da amostra da coorte utilizaram os seviços odontológicos no último ano. Os indivíduos que visitaram o dentista por motivo preventivo, em consulta privada, que estavam muito satisfeitos ou satisfeitos com sua saúde bucal utilizaram em maior proporção esses serviços. Além disso, o maior índice de CPOD também levou ao maior uso.

Resumo Estudo exploratório de abordagem qualitativa que buscou analisar os conhecimentos, percepções e práticas de equipes de Saúde Bucal e de agentes comunitários de saúde acerca das políticas públicas que regulamentam a utilização de plantas medicinais e fitoterápicos no contexto das práticas integrativas e complementares, incluindo sua incorporação, reconhecimento, desafios e possibilidades no cotidiano local do SUS. Foram selecionadas oito Unidades Básicas de Saúde no município de Itabuna, Bahia, Brasil, com a realização de entrevistas com oito cirurgiões-dentistas, oito auxiliares/técnicos de saúde bucal e oito agentes comunitários de saúde, totalizando 24 participantes. Para a análise da dados, empregou-se a técnica do Discurso do Sujeito Coletivo. Os conhecimentos, percepções e práticas dos atores envolvidos no estudo sobre as políticas públicas de inserção das plantas medicinais no SUS mostraram-se permeados pela cultura hegemônica, pela formação em saúde tradicional e fragmentada, pelo (des)interesse administrativo do município e por subjetividades individuais que são produzidas e reproduzidas nos serviços de saúde bucal da APS local.


Abstract This exploratory and qualitative study aimed to analyze the knowledge, perceptions and practices of Oral Health Teams and Community Health Agents about public health policies that regulate the use of medicinal plants and herbal medicines in the context of integrative and complementary practices, including their incorporation, recognition, challenges and possibilities in the local routine of the Brazilian Unified Health System (SUS). Eight Basic Health Units were selected in the municipality of Itabuna, Bahia, Brazil, and interviews were conducted with 8 dentists, 8 oral health assistants/technicians and 8 community health agents, totaling 24 participants. For data analysis, the Collective Subject Discourse technique was used. The knowledge, perceptions and practices of the actors involved in the study about public policies for the introduction of medicinal plants in the SUS were permeated by the hegemonic culture, traditional and fragmented health education, the administrative (un)interest of the municipal authorities, and individual subjectivities that are produced and reproduced in the local oral health services and the primary care.

ABSTRACT

Objetivo: reconocer la asociación entre el acceso a atención odontológica y la autopercepción de salud bucal, entendida como buena, regular o mala, según valoración subjetiva, en mujeres adultas trabajadoras sexuales. Métodos: scoping review durante octubre de 2022 en EMBASE (Elsevier), LILACS y PUBMED. Se tuvo en cuenta la guía de métodos de Joanna Briggs Institute (JBI), así como la extensión PRISMA para Scoping Reviews (PRISMA-ScR). Se incluyeron estudios de corte transversal que incluían como actores de estudio a mujeres trabajadoras sexuales y los temas que aquí se abordan. La calidad de los estudios se evaluó mediante la herramienta New Castle-Ottawa. Resultados: se identifcaron 48 artículos de los cuales tres fueron finalmente incluidos en la síntesis de los resultados. La población de los estudios incluidos estuvo centrada en trabajadoras sexuales de diferentes lugares como Suiza, China e India, alrededor de las cuales se abordó el acceso a servicios de atención odontológica y la autopercepción de salud; igualmente, fue común el abordaje detemas de salud sexual. Se evidenciaron barreras económicas, sociales, culturales en el acceso a atención odontológica en trabajadoras sexuales, asimismo la salud autopercibida de las mismas fue interpretada como buena. Conclusión: se evidenció una limitada cantidad de información sobre el acceso a servicios de salud bucal por parte de trabajadoras sexuales con respecto a la información disponible en salud sexual. Por este motivo, es relevante realizar más estudios que tengan en cuenta el componente de salud bucodental en trabajadoras sexuales, teniendo en cuenta los riesgos y vulnerabilidades a las que se encuentran expuestas.


Objective: To recognize the association between access to dental care and self-perception of oral health in terms of subjective assessment as good, regular/average, or bad/poor, in adult women sex workers. Methods: A scoping review during October2022 in EMBASE (Elsevier), LILACS and PUBMED. This study takes into considerationThe Joanna Briggs Institute (JBI) method guide and the PRISMA extension for Scoping Reviews (PRISMA-ScR). The current study also includes cross-sectional studies that considered variables like sex workers, access to dental care, and self-perceived health. The quality of the studies was evaluated using the New Castle-Ottawa tool. Results: Forty-eight articles were identifed, of which three were fnally included in the synthesis of the results. The population of the included studies was focused on sex workers from diferent places such as Switzerland, China and India, the issue of access to dental care services and self-perception of health was also common addressing sexual health issues, these studies identifed economic, social, and cultural barriers in access to dental care for sex workers; also, their self-perceived health was interpreted as good. Conclusion: The limited amount of information regarding access to oral health services by sex workers, regarding information found in terms of sexual health, was evidenced; for this reason, it is important to carry out more studies that consider the oral health component in sex workers, considering the risks and vulnerabilities to which they are exposed.

Abstract This ecological study assessed the impact of the COVID-19 pandemic on completed treatments (CTs) and referrals during urgent dental visits to primary health care units in Brazil, and their associations with socioeconomic, geodemographic, and pandemic index factors in Brazilian municipalities. The difference in rates of procedures 12 months before and during the pandemic was calculated. Data were extracted at baseline from health information systems of all municipalities that provided urgent dental care (n = 5,229 out of 5,570). Multiple logistic regression predicted the factors associated with referrals and CTs. The number of dental urgencies increased from 3,987.9 to 4,272.4 per 100,000 inhabitants. The rates of referrals decreased in 44.1% of the municipalities, while 53.9% had lower rates of CTs. Municipalities with a greater number of oral health teams in the primary health care system (OR = 1.52, 95%CI:1.21-1.91) and with specialized services (OR = 1.80, 95%CI:1.50-2.16) were more likely to decrease referrals during the pandemic. Higher HDI and GDP per capita were associated with a larger decrease in referrals and smaller decrease in CTs. The calamity generated by the long pandemic period resulted in a greater demand for urgent visits. Less developed and larger cities seem to have been more likely to not complete treatments during urgent visits in primary dental care units in times of calamity. Primary dental care offices in smaller and less developed municipalities should be better equipped to provide appropriate assistance and to improve the problem-solving capacity of dental services during emergencies.

Resumo: No Brasil, houve expansão da cobertura de serviços odontológicos na atenção primária à saúde (APS), e a ênfase do trabalho dos profissionais mudou para incluir mais esforços na prevenção e no diagnóstico. Entretanto, pouco se sabe sobre a influência da cobertura do Programa Bolsa Família no uso desses serviços. Esta pesquisa avaliou a associação entre cobertura municipal do Programa Bolsa Família e uso de serviços odontológicos. Este estudo ecológico, realizado com dados dos 5.570 municípios brasileiros, estimou, por meio de regressões logísticas, o impacto da variação de cobertura do Programa Bolsa Família, das Estratégias Saúde da Família (ESF) e das equipes de saúde bucal (EqSB) no número de procedimentos odontológicos restauradores, coletivos, preventivos e exodontias realizados via Sistema Único de Saúde (SUS) entre os períodos 2007/2008 e 2010/2011. Os percentuais de municípios em que houve aumento das taxas de procedimentos preventivos, coletivos, restauradores e exodontias foram de 46%, 59,8%, 52,5% e 44,2%, respectivamente. No modelo ajustado, em municípios com maior cobertura do Bolsa Família houve menos chances de aumentar a ocorrência de procedimentos coletivos (OR = 0,91; IC95%: 0,79-1,04) e preventivos (OR = 0,92; IC95%: 0,80-1,05) e mais chances de elevar as taxas de procedimentos restauradores (OR = 1,11; IC95%: 0,97-1,28) e exodontias (OR = 1,10; IC95%: 0,95-1,27). A expansão na taxa de cobertura das EqSB esteve associada significativamente a uma chance maior de aumento do número de procedimentos preventivos, restauradores e exodontias. Conclui-se que a cobertura das EqSB foi a principal variável associada à ampliação da quantidade de procedimentos odontológicos realizados no serviço público.


Abstract: In Brazil, there has been an expansion of the coverage of dental services in primary health care (PHC), and the focus of the services has changed to include more efforts in prevention and diagnosis. However, little is known about the influence of the coverage of the Brazilian Income Transfer program on the use of dental services. Our study evaluates the association between municipal coverage of the Brazilian Income Transfer Program and the use of dental services. This ecological study conducted with data from the 5,570 Brazilian municipalities estimates, via logistic regressions, the impact of the coverage of the Brazilian Income Transfer program, the Family Health Strategies (FHS), and the oral health teams on the number of restoration, colective, prevention, and dental extraction procedures performed by the Brazilian Unified National Health System (SUS) between the periods 2007/2008 and 2010/2011. The percentage of municipalities that increased the rates of prevention, colective, restoration, and extraction procedures was 46%, 59.8%, 52.5%, and 44.2%, respectively. In the adjusted model, municipalities with increased coverage of Brazilian Income Transfer program were less likely to increase colective (OR = 0.91; 95%CI: 0.79-1.04) and prevention (OR = 0.92; 95%CI: 0.80-1.05) procedures, and they were more likely to increase rates of restoration (OR = 1.11; 95%CI: 0.97-1.28) and dental extraction (OR = 1,10; 95%CI: 0.95-1.27) procedures. The increase in the coverage rate of the oral health teams was significantly associated with a higher chance of an increase in prevention, restoration, and extraction procedures. We conclude that the coverage of oral health teams was the main variable associated with the increase in dental procedures performed in the public service.


Resumen: En Brasil, hubo una expansión de la cobertura de servicios odontológicos en la atención primaria a la salud, y el énfasis del trabajo de los profesionales cambió para incluir más esfuerzos en la prevención y en el diagnóstico. Sin embargo, poco se sabe sobre la influencia de la cobertura del Programa Bolsa Familia en el uso de los servicios odontológicos. El presente estudio evaluó la asociación entre la cobertura municipal del Programa Bolsa Familia y uso de servicios odontológicos. Un estudio ecológico realizado con datos de los 5.570 municipios brasileños estimó, a través de regresiones logísticas; el impacto de la variación de cobertura del Programa Bolsa Familia, de las Estrategias de Salud de la Familia (ESF) y de los equipos de salud bucal (EqSB) en el número de procedimientos odontológicos restauradores, colectivos, preventivos y exodoncias realizados a través del Sistema Único de Salud (SUS) entre los períodos 2007/2008 y 2010/2011. El porcentaje de municipios que aumentaron las tasas de procedimientos preventivos, colectivos, restauradores y exodoncias fue del 46 %, 59,8 %, 52,5 % y 44,2 %, respectivamente. En el modelo ajustado, los municipios con aumento de la cobertura de Programa Bolsa Familia tuvieron menos probabilidades de aumentar procedimientos colectivos (OR = 0,91; IC95%: 0,79-1,04), y preventivos (OR = 0,92; IC95%: 0,80-1,05), y más probabilidades de aumentar las tasas de procedimientos restauradores (OR = 1,11; IC95%: 0,97-1,28) y exodoncias (OR = 1,10; IC95%: 0,95-1,27). El aumento en la tasa de cobertura de EqSB se asoció significativamente con una mayor probabilidad de aumento en procedimientos preventivos, restauradores y exodoncias. Se concluye que la cobertura de las EqSB fue la principal variable asociada al aumento de los procedimientos odontológicos realizados en el servicio público.

@#The coronavirus disease 2019 (COVID-19) has sparked concerns among dental patients due to its high infectivity. The implementation of the Movement Control Order in 2020 by the Malaysian government (MMCO-2020) has added constraints to access to dental clinics and may affect the patients’ status of oral disease. This study aims to investigate the impact of MMCO-2020 on the periodontal health and oral health-related quality of life (OHRQoL) of periodontitis patients with/out non-communicable diseases (NCDs). A cross-sectional study involving 139 patients with periodontitis was conducted to assess patients’ OHRQoL, oral health problems, and their perceived barriers to access dental treatment during MMCO-2020 using the Oral Health Impact Profile (OHIP-14) and questionnaire on oral health-related problems. The overall mean OHIP-14 score was relatively low (10.08 ± 8.861), suggesting that the OHRQoL is generally good during MMCO-2020. However, 44 (31.7%) periodontitis patients score ADD ≥14, indicating poorer oral health status and greater dental treatment needed. There was statistically significant association between periodontitis patients with comorbidities (i.e., presence of NCDs) with poorer OHIP-14 scores and mean plaque score changes at pre- and postMMCO, suggesting that special care is required by this group, even during the national lockdown. Of the 45 (32.4%) patients who reported having problems, most (30, 36.1%) decided to “visit the dentist for treatment’. Meanwhile, the most frequent barriers were “could not get an appointment” (14, 34%), “problem is not serious” (13, 39.4%) and “fear of COVID” (9, 27.3%). The MMCO-2020 restrictions on the population had a minimal negative impact on the OHRQoL of patients with periodontal disease, but provisions can be arranged to address the basic and urgent needs of selected periodontitis patients, especially those presented with medical comorbidities.

RESUMO O objetivo deste estudo transversal foi identificar adequações quanto à utilização de Equipamentos de Proteção Individual e organização do processo de trabalho no atendimento odontológico ambulatorial no Paraná durante o primeiro ano da pandemia da Covid-19. A amostra de conveniência incluiu 1.105 profissionais de saúde bucal (cirurgiões-dentistas, técnicos e auxiliares em saúde bucal) durante o primeiro ano da pandemia (agosto a outubro de 2020). Formulário on-line (Google Formulários®) foi enviado por e-mail pelo Conselho Regional de Odontologia. As medidas de enfrentamento da Covid-19 foram comparadas entre os tipos de serviços odontológicos: ambulatorial do Sistema Único de Saúde - SUS (Atenção Primária, Centro de Especialidades Odontológicos e Pronto Atendimento), clínica privada, e outros (Sistema S, forças de segurança, sindicatos, hospitais públicos e privados e clínicas de ensino). Os profissionais dos serviços ambulatoriais do SUS relataram com maior frequência que: suspenderam atendimentos eletivos, evitavam gerar aerossóis e trabalhavam a quatro mãos. Nas clínicas privadas, utilizavam frequentemente teleorientação e telemonitoramento. Nos 'outros serviços', houve maior proporção de redução das horas de trabalho e autoclavagem de peças de mão após cada atendimento. Conclui-se que, apesar da alta adequação às medidas de enfrentamento da Covid-19, houve diferença na prática profissional nos diferentes tipos de serviços odontológicos.


ABSTRACT This cross-sectional study aimed to identify adjustments regarding the use of Personal Protective Equipment and the organization of the work process in outpatient dental care in Paraná during the first year of the COVID-19 pandemic. The convenience sample included 1,105 oral health professionals (dental surgeons, oral health assistants, and dental hygienists) during the first year of the pandemic (August to October, 2020). An online form (Google Forms®) was sent by email by the Regional Council of Dentistry. COVID-19's measures were compared between the types of dental services: 'outpatient clinic of the Unified Health System - SUS' (Primary Care, Dental Specialty Center, and Emergency Care), 'private clinic' and 'other services' (Sistema S, security forces, trade union, public and private hospitals, and teaching clinics). Professionals from SUS outpatient services reported more frequently: that they suspended elective care, avoided generating aerosols, and worked four-handed. Professionals from 'private clinics' used teleorientation and telemonitoring more regularly. In the 'other services', there was a more significant proportion reduction in work hours and handpieces autoclaving after each service. It is concluded that, despite the high adequacy of the measures to combat COVID-19, there was a difference in professional practice among the different types of dental services.

Introdução:Uma ampla gama de fatores pode contribuir para facilitar ou restringir o uso de serviços de saúde bucal pela população. A compreensão desses fatores pode contribuir para a identificação das parcelas da população com maior dificuldade de acesso e auxiliar na elaboração de políticas públicas de saúde voltadas para populações específicas de forma equânime. Objetivo:Analisar a produção científica acerca dos fatores associados ao uso de serviços odontológicos públicos no Brasil.Metodologia:Foi realizada uma revisão integrativa da literatura, com busca de artigos originais publicados entre 2011 e 2021, nas bases Medline, Lilacs, SciELO e BVS. De um total de 724 estudos, 10 artigos atenderam aos critérios de elegibilidade propostos e foram selecionados para a revisão.Resultados:entre as crianças os fatores associados ao uso de serviços odontológicos públicos foram: condição socioeconômica, raça, escolaridade da mãe e necessidade de tratamento. Nos adultos: gênero, raça, renda, nível de escolaridade, histórico de dor de dente, cárie, avaliação do tratamento recebido como regular, autopercepção de saúde bucal e de necessidade de tratamento. E entre idosos: raça, renda, nível de escolaridade, uso de serviços para fins curativos, uso de prótese e autopercepção da saúde bucal. Conclusões:Sugerem-se estudos longitudinais para elucidação de relações de causalidade e estudos com a população adolescente. São necessárias mudanças na assistência odontológica no país, de forma a superar a perspectiva focalizada de um SUS para pobres e alcançar uma Atenção à Saúde Bucal baseada nos princípios da universalidade, integralidade e equidade (AU).


Introduction:A wide range of factors can contribute to facilitate or to limit the use of oral health services by people. The understanding of these variables can contribute to identify the segments of the population with more difficulty of access and help the development of public health policies related to specific groups of people evenly. Objective:This paper aims to analyze the scientific production about the factors associated with the public dental services in Brazil. Methodology:An integrative review about the literature in the area was conducted, in search for papers published between 2011 and 2021, in Medline, Lilacs, SciELO and BVS. From 724 studies, 10 researches fulfilled the proposed eligibility criteria and were selected to the review. Results:Among the children, the factors associated with the use of public dental services were: socioeconomic condition, race, mother ́s educational degree and the need for treatment. In the case of adults: gender, race, income, level of education, medical history of toothache and dental caries, regular evaluation of the treatment received, the self-perception of oral health and of the need of treatment. Among the elderly people: race, income, level of education, the use of services for curative purposes, the use of prosthesisand the self-perception of oral health. Conclusions:It is suggested the development of longitudinal studies in order to elucidate the relations of causality and of studies with the teenagers. Changes in dental assistance in the country are necessary, in order to overcome the perspective of a SUS to poor people and to reach a Primary Dental Healthcare based in the principles of universality, integrality and equity (AU).


Introducción: Una amplia gama de factores puede contribuir a facilitar o restringir el uso de los servicios de salud bucal por parte de la población. La comprensión de estos factores puede contribuir a la identificación de las partes de la población con mayor dificultad de acceso y ayudar en la elaboración de políticas públicas de salud dirigidas a poblaciones específicas de manera equitativa. Objetivo: Analizar la producción científica sobre factores asociados al uso de los servicios públicos odontológicos en Brasil. Metodología: Se realizó una revisión integrativa de la literatura, buscando artículos originales publicados entre 2011 y 2021, en las bases de datos Medline, Lilacs, SciELO y BVS. De un total de 724 estudios, 10 artículos cumplieron con los criterios de elegibilidad propuestos y fueron seleccionados para revisión. Resultados: Entre los niños, los factores asociados al uso de los servicios odontológicos públicos fueron: nivel socioeconómico, raza, nivel de escolaridad de la madrey necesidad de tratamiento. En adultos: género, raza, ingresos, nivel de escolaridad, antecedentes de dolor de dientes, caries, evaluación del tratamiento recibido como habitual, autopercepción de la salud bucal y de necesidad de tratamiento. Y entre los adultos mayores: raza, ingresos, nivel de escolaridad, uso de servicios con fines curativos, uso de prótesis y autopercepción de la salud bucal. Conclusiones: Se sugieren estudios longitudinales para aclarar las relaciones causales y estudios sobre la población adolescente. Se necesitan cambios en la atención odontológica en el país, a fin de superar la perspectiva focalizada de un SUS para los pobres y lograr una Atención de Salud Bucal basada en los principios de la universalidad, integralidad y equidad (AU).

Resumo Introdução Há escassos estudos sobre qualidade da atenção em saúde bucal. Objetivo Avaliar associações entre condições de estrutura e processo com indicadores de desempenho em saúde bucal na Estratégia Saúde da Família do Recife. Métodos Estudo transversal, quantitativo e avaliativo, usando dados secundários de 2014 do Ministério da Saúde. Foram calculados testes de Qui-Quadrado e estimativas de Odds Ratio, com regressão logística simples e múltipla, considerando nível de significância de 5% e intervalo de confiança de 95%. Resultados Das 112 equipes de saúde bucal (ESB) avaliadas, equiparação das ESB com as equipes mínimas da Estratégia Saúde da Família e possuir levantamento de escolares com necessidades de saúde bucal, associaram-se ao maior valor de cobertura de primeira consulta odontológica programática (OR = 4,30, IC = 1,33−13,93; OR = 27,47, IC = 2,24−336,34, respectivamente), enquanto estar equiparada, ser ESB modalidade II e garantir agendamento de retorno para a continuidade do tratamento associaram-se ao maior valor da razão entre tratamentos concluídos e primeiras consultas odontológicas programáticas (OR = 5,35, IC = 2,01−14,20; OR = 3,70, IC =1,33−10,27; OR = 5,03, IC = 1,42−17,78, respectivamente). Conclusão Os resultados sugerem que tais padrões de qualidade sejam priorizados para potencializar maior acesso e resolutividade das ESB.


Abstract Background Only a few studies have addressed the quality of oral health care. Objective To assess the association of the structure and process conditions with oral health performance indicators in the Family Health Strategy of Recife. Method A cross-sectional quantitative and evaluative study based on secondary data referring to 2014 from the Ministry of Health. The Chi-Square test and the Odds Ratio, with simple and multiple regression, were calculated considering a significance level of 5% and confidence interval of 95%. Results Among the 112 oral health teams (OHT) assessed, there was an association of the OHT parity with the minimum Family Health Strategy teams and the survey of schoolchildren with oral health needs with the highest coverage value of the first dental programmatic appointment (OR = 4.30 = CI = 1.33-13.93, OR = 27.47, IC = 2.24-336.34, respectively). In turn, the presence of parity, modality II OHT, and scheduled guarantee return for the treatment continuity were associated with the highest ratio between completed treatment and first dental appointment (OR = 5.35, CI = 2.01-14.20; OR = 3.70, CI = 1,33-10,27; OR = 5,03, CI = 1,42-17,78, respectively). Conclusion The results suggest the targeting of such quality standards to enhance both the access and resilience of the OHT.

RESUMEN Fundamento: Las prótesis estomatológicas son una solución estética y funcional a la pérdida de los dientes y contribuyen a la calidad de vida de aquellos que la portan, pero el sistema estomatognático se expone a lesiones cuando no se cumplen las orientaciones para su uso, cuidado e higiene. Objetivo: Evaluar la efectividad del programa educativo para mejorar la salud bucal en pacientes portadores de prótesis estomatológicas parciales. Metodología: Se realizó un estudio preexperimento con medición antes y después de la intervención comunitaria sin grupo de control, en el período de septiembre 2019 a febrero del 2020. Se seleccionaron 50 pacientes mayores de 19 años portadores de prótesis estomatológica parcial, que cumplieron criterios de inclusión. Se utilizó la encuesta, la observación, el criterio de especialistas y el experimento como métodos del nivel empírico y estadístico-matemático para medir las variables: nivel de conocimiento, eficiencia del cepillado bucodental e higiene de las prótesis estomatológicas. Resultados: Al culminar la aplicación del programa educativo se observó un cambio de 6 % a un 52 % del nivel de conocimiento bueno sobre el uso y cuidado de las aparatologías protésica, en la eficiencia del cepillado de un 30 % a un 82 % y de un 8 % a un 68 % de buena higiene en las prótesis. Conclusiones: El programa educativo se evaluó de efectivo al obtener resultados satisfactorios en cuanto a la modificación de los conocimientos y prácticas sobre salud bucal en los pacientes portadores de prótesis estomatológicas.


ABSTRACT Background: Dentures are an esthetic and functional solution to tooth loss and contribute to life quality of those who wear them, but the stomatognathic system is exposed to lesions when guidelines for their use, care and hygiene are not followed. Objective: To assess the effectiveness of an educational program to improve oral health in patients with partial dentures. Methodology: A measurement pre-experiment study was conducted before and after community intervention without any control group, from September 2019 to February 2020. 50 patients over 19 years of age wearing partial denture who met the inclusion criteria were selected. Survey, observation, specialist criteria and experiment were used as empirical and statistical-mathematical methods to measure variables: knowledge level, oral brushing efficiency and dentures hygiene. Results: At the end of the application of the educational program, a change was observed from 6 % to 52 % in the good knowledge level on wear and care of prosthetic appliances, in brushing efficiency from 30 % to 82 % and from 8 % to 68 % in prostheses good hygiene. Conclusions: The educational program was assessed as effective in obtaining satisfactory results in terms of modification of oral health knowledge and practices in patients with dentures.

ABSTRACT

Com o surgimento da pandemia causada pelo SARS-Cov-2, houve a necessidade de reestruturação dos serviços de saúde no Brasil, incluindo a clínica odontológica. O objetivo do presente estudo é descrever as medidas de biossegurança realizadas ou modificadas na prática de saúde bucal no estado do Paraná, em nível público e privado, envolvendo três categorias profissionais: cirurgiões-dentistas (CD), técnicos em saúde bucal (TSB) e auxiliares de saúde bucal (ASB). Esta pesquisa transversal descritiva foi realizada nos meses de agosto a outubro de 2020, com o envio de questionários on-linepor e-maile divulgação em redes sociais. Foram envolvidos 1072 profissionais, sendo 75,6% CD, 16,3% ASB e 8,1% TSB. O gênero feminino foi predominante entre os participantes (81,1%), com 46,1% apresentando idade entre 40 e 59 anos. Gorro, máscara cirúrgica, óculos de proteção e protetor facial são referidos como sempre utilizados, respectivamente, por 92,3%, 81,0%, 80,0% e 79,1% dos participantes. A máscara N95/PFF2 (67,5%) e o avental impermeável (20,3%) tiverem os maiores percentuais de "nunca disponíveis ou utilizados". Cerca de 50% afirmaram que tiveram acesso à Nota Técnica GVIMS/GGTES/ANVISA Nº 04/2020. Os profissionais avaliados, em geral, realizaram mudanças em suas práticas de biossegurança, mas precisam ser acompanhados durante toda a extensão da pandemia.


With the emergence of the SARS-Cov-2 pandemic, there was a need to restructure health services in Brazil, including the dental clinic. The aim of the present study was to describe the biosafety measures performed or modified in oral health practice at public and private levels, involving the three professional categories: dental surgeons (DDS), oral health technicians(OHT), and health assistants (HA) in the state of Paraná. This escriptive cross-sectional survey was carried out from August to October 2020, with an online questionnaire being sent by email and published on social networks. There were 1072 professionalsinvolved, of which 75.6% were DDS, 16.3% were OHT, and 8.1% were HA. The female gender was predominant among the participants (81.1%), and 46.1% were aged between 40 and 59 years. 92.3%, 81.0%, 80.0%, and 79.1% of the articipants, respectively, said theyalways used a cap, surgical mask, protective glasses, and face shield. The N95/PFF2 mask (67.5%) and the waterproof apron (20.3%) are two of the most frequently mentioned items as never being available or used. About 51% said they had access to Technical Note GVIMS/GGTES/ANVISA No. 04/2020. In general, the professionals evaluated have been making changes in their biosafety practices, but they still need to be monitored throughout the pandemic.

RESUMEN Introducción: el personal de salud involucrado en la atención odontológica tiene alto riesgo de contagio por exposición constante a aerosoles durante la atención de urgencias. Es necesario incrementar sus medidas de protección personal durante la pandemia por SARSCoV-2. Objetivo: describir la experiencia de odontólogos, auxiliares y pacientes respecto al uso de un dispositivo de barrera para contención de aerosoles en la atención de urgencias odontológicas. Materiales y métodos: estudio descriptivo de corte transversal con 21 pacientes adultos atendidos por urgencias odontológicas en una institución privada durante el mes de junio en la ciudad de Bogotá. Los 21 pacientes, cinco odontólogos tratantes y cinco auxiliares de odontología diligenciaron un cuestionario estructurado sobre la experiencia con el dispositivo, medidas de bioseguridad implementadas y percepción de seguridad respecto al posible contagio del virus. Resultados: la edad promedio de los pacientes fue 41 años (DE±14). La duración de los procedimientos fue de 45 minutos (DE±12) en promedio. No hubo eventos adversos durante los procedimientos con el uso del dispositivo. El 95 % de los pacientes se sintieron más seguros durante la atención con el uso del dispositivo de barrera. Conclusiones: a pesar de la muestra limitada, la alta percepción de seguridad respecto al uso del dispositivo en la atención de urgencias odontológicas reportada aquí, sugiere que pueden llegar a complementar las medidas de bioseguridad y equipos de protección personal establecidos por autoridades sanitarias durante la pandemia.


SUMMARY Introduction: Health personnel involved in dental care are at high risk of contagion from constant exposure to aerosols during emergency care. It is necessary to increase its protection measures during the SARS-CoV-2 pandemic. Objective: Describe the experience of dentists, assistants and patients regarding the use of a barrier device to contain aerosols in dental emergency care. Materials and methods: A descriptive cross-sectional study with 21 adult patients attended by dental emergencies in a private institution during June in the city of Bogotá. The 21 patients, five treating dentists and five dental assistants filled out a structured questionnaire about the experience with the device, the biosafety measures implemented and the perception of safety regarding the possible contagion of the virus. Results: The mean age of the patients was 41 years (SD ± 14). The duration of the procedures was 45 minutes (SD ± 12) on average. There were no adverse events during the procedures with the use of the device. 95% of patients felt safer during care with the use of the barrier device. Conclusions: Despite the limited sample, the high perception of safety regarding the use of the device in dental emergency care reported here suggests that they may complement the biosecurity measures and personal protective equipment established by health authorities during the pandemic.

O último levantamento epidemiológico em Saúde Bucal (Projeto SB-Brasil 2010) apresentou uma distribuição desigual e alta prevalência da doença cárie na população brasileira em crianças aos 5 anos. Desde a criação do SUS, que a oferta das ações e serviços em saúde tem o objetivo de serem empregadas atendendo os princípios: Universalidade, Integralidade e Equidade, implementados através da Lei 8.080/90. A oferta de uma atenção integral está direcionada a atender os aspectos físicos, mentais e sociais do indivíduo. Desta forma, é fundamental que a APS ofereça uma diversidade de cuidados, entre eles a atenção à saúde bucal. Uma assistência integral para crianças em idade pré-escolar é primordial, dessa maneira a atenção odontológica colabora para a prevenção de doenças bucais, principalmente a cárie, e consequentemente na manutenção dos dentes que influenciam diretamente no crescimento e desenvolvimento crânio facial, na oclusão, na função mastigatória e fonoarticulatória. O presente estudo objetivou avaliar a qualidade das ações e serviços odontológicos na APS para crianças de 0 a 5 anos no estado de Minas Gerais. Tratase de um estudo transversal, descritivo e analítico, cujo a coleta de dados foi realizada através do instrumento PCATool versão profissionais. Dos 853 municípios de Minas Gerais, 196 municípios participaram do estudo, representados pelo coordenador de saúde bucal. A análise descritiva dos dados mostrou que 86,2% dos municípios apresentam os atributos essenciais e derivados para a APS, apresentando qualidade das ações e serviços em saúde bucal para crianças de até 5 anos. O atributo acesso de primeiro contato, acesso e acessibilidade apresentou menor percentual. Apenas, 17,8% dos municípios apresentaram resultados acima da pontuação de corte estabelecida pelo instrumento PCATool. Os resultados desse estudo foram correlacionados ao IMRS, e não apresentaram relação. Apesar da grande extensão territorial do estado de Minas Gerais a distribuição da qualidade das ações e serviços em saúde bucal foi similar para as 13 regiões geográficas. As justificativas para esses resultados podem estar vinculadas ao incentivo financeiro oferecido pelo governo de Minas Gerais, que atua com o objetivo de expandir os cuidados primários, principalmente em municípios que apresentam uma maior vulnerabilidade social. Os resultados encontrados demonstram uma expressiva falta de acesso aos serviços, o que pode influenciar além de outros fatores, como a percepção sobre a saúde bucal, a alta prevalência de cárie em crianças. Contudo, esse estudo evidência que quando a criança consegue acesso aos serviços de saúde bucal esses apresentam uma condição satisfatória quanto a estrutura e processo dos serviços capazes de garantir a prevenção de doenças e recuperação da saúde. Estudos como esse são necessários uma vez que permitem criar ações e estratégias para a melhoria da qualidade dos serviços ofertados.


The last epidemiological survey on Oral Health (SB-Brazil Project 2010) showed an unequal distribution and high prevalence of caries disease in the Brazilian population of children at 5 years old. Since, the creation of SUS, the offer of health actions and services has the objective of being employed according to the principles of SUS: Universality, Integrality, and Equity, implemented by Law 8.080/90. The offer of comprehensive care is directed to meet the physical, mental, and social aspects of the individual. Thus, it is fundamental that PHC offers a diversity of care, including oral health care. Comprehensive care for preschool children is essential, thus dental care contributes to the prevention of oral diseases, especially caries, and consequently in the maintenance of teeth that directly influence the growth and craniofacial development, occlusion, chewing, and phono-articulatory function. Therefore, the present study aimed to evaluate the quality of dental actions and services in PHC for children aged 0 to 5 years in the state of Minas Gerais. This is a cross-sectional, descriptive, and analytical study, whose data collection was performed using the instrument PCATool professional version. In all, from the 853 municipalities of Minas Gerais, 196 municipalities participated in the study represented by the oral health coordinator. The descriptive analysis of the data showed that 86.2% of the municipalities have the essential and derived attributes for PHC, presenting the quality of actions and services in oral health for children up to 5 years. The attribute first contact access, access, and accessibility showed a lower percentage, only 17.8% of the municipalities showed results above the cut-off score established by the PCATool instrument. The results of this study were correlated to the IMRS and showed no relationship. Despite the large territorial extension of the state of Minas Gerais, the distribution of the quality of oral health actions and services was similar for the 13 geographic regions. The justifications for these results may be linked to the financial incentive offered by the government of Minas Gerais that acts to expand primary care, especially in municipalities that present greater social vulnerability. The results found show a significant lack of access to services, which may influence, besides other factors such as perception of oral health, the high prevalence of caries in children. However, this study shows that when children have access to oral health services, these have a satisfactory conditions regarding the structure and process of services able to ensure the prevention of diseases and recovery of health. Studies like this are necessary since they allow the creation of actions and strategies to improve the quality of the services offered.
